Getting Started:

To schedule your initial evaluation please follow the steps outlined below. The evaluation will combine the best of both "high touch" and "high tech" and will take approximately one hour.

Call our office at 687-7775 to reserve your appointment
Please have your insurance information ready so we may check your benefits prior to your first visit
Download and complete the appropriate health history and HIPPA forms that are shown below

Your insurance card
