Getting Started:
To schedule your initial evaluation please follow the steps outlined below. The evaluation will combine the best of both "high touch" and "high tech" and will take approximately one hour.
- Call our office at 687-7775 to reserve your appointment
- Please have your insurance information ready so we may check your benefits prior to your first visit
- Download and complete the appropriate health history and HIPPA forms that are shown below

- Your completed health history and HIPPA forms (see below)
- Your insurance card
